FORMS INSTRUCTIONS
Team Registration Forms should be faxed to 702.248.9445
Individual Athlete Enrollment Forms should be collected by the Team Manager and mailed or delivered to the
League office.

Individual Athlete Tournament Registration Forms, and payment, should be collected by the Team Manager
and brought with you on Tournament day. A Roster of your Team's competing athletes, coaches and
volunteers must be FAXED or emailed to the League at least 2 days prior to the Tournament.
